This is a simple birth plan template that you can use to communicate with your birth team. Ideally, birth plans should be discussed with your birth team before your labor begins. Choose a place of birth and a team that supports you. Not writing a birth plan doesn’t mean you don’t have one. It just means your birth plan was written by someone else, your health care provider and place of birth in the form of policies, standard practices, habits and standing orders.
